This form is to confirm team nominations for the upcoming season. It is a requirement that teams are set up in Dribl with at least 11 players assigned (9 players for U12).

Hosting Friday Night Matches
Clubs who have lodged a certified lux certificate of 100 lux or more to the Association may nominate to host Friday night competition fixtures at their ground on the condition that FSC allocates, at their discretion, any of the home teams from U12 - U19.
